Checklist is one of main practice that had been use by many organization especially organization that involved with programming. It had been proven very useful in assisting organization to achieve its target. It is also well-known for its simple features and low cost of implementation. Many had labelled that checklist is a great tool that helped them to achieve their target. It is often heard that in some occasion, organization did produce defects. Some of this defect had prevention steps that already mentioned in the checklist. This raised a major question among its practitioner. They questioned if checklist is already efficient. Some of them also question if it is fool prove. Therefore, there are immediate needs to find the solution and source of these defects. Purpose of this research is to investigate if the checklist is really working as many people thought it would be. Factor such as attitude, human behaviour, long list, weak enforcement, complex technical term, urgency of request, experience, document version, changed of process and misinterpretation need to be investigate to prove this claim. To achieve this target, Mix method approach had been used. Survey and interview in a single case study had been done. This involved a population of 70 programmer and developer within a well-known international company in Cyberjaya. A sum of 46 Correspondents had participated in the survey study and 3 interviewees had contributed their feedback during these studies. Resulted from this research, it is found out that the entire factor which had been mentioned above is vague in some area. It is found out that long checklist did not have effects on packager/programmer during research. Further study should be done on this area. A study should be done on how to improve the efficiency of checklist in the future research recommendation.
