Near misses dispensing errors in Out Patient Department (ACC) at Hospital Sultanah Nurzahirah K.Terengganu

Introduction:
Near misses dispensing error included any inconsistencies or deviations from the prescriptions order such as dispensing the incorrect drug, dose, dosage form; wrong quantity; inappropriate, incorrect, or inadequate labeling, confusing, or inadequate directions for medication use; incorrect or inappropriate preparation, packaging, or storage or medication prior to dispensing.
Objective:
The main objectives of this study are to determine the frequency of different types of dispensing errors, to explore the reasons why they occurred, to determine the near misses dispensing errors which occur per month and to evaluate the effects that might be happened.
Methodology:
This is a retrospective study and the data was collected from JAN 2008 to DEC 2009 in out patient department (ACC) HSNZ using "BORANG QAP 3 (daily recording of errors detected before dispensing) was used as our method of data collection. Furthermore questionnaire on pharmacy staffs opinion towards near misses dispensing errors were randomly distributed. We use SPSS 15 to analyse all data that we collected.
