Abstract
The compliance level towards TDM request form has been explored. This study focus on how does the compliance of TDM request form significantly affect on the appropriateness of the TDM result. This included the sampling procedure, method of analytic and pharmacist recommendation. Hospital Tengku Ampuan Rahimah (HT AR) Klang have been chosen to carry this study. Obviously it is the biggest and oldest hospital in Selangor. After all this hospital run TDM by pharmacy-based compared to some other hospital that may conducted by lab-based. Therefore, the study subject is previous TDM request form filled by medical officer of HTAR to run the test. Those form are chosen randomly between of January 2014 till June 2014 and have been recorded on our designed data collection form. This study could relate the significant of demographic factor, clinical information and patient condition that may effects the outcome result especially on pharmacist further recommendation and interpretation. From the 200 data collected, we are manage to identify the level of compliance of TDM request form by the medical officer in HT AR, Klang. As expected the level of compliance is slightly poor and by using the Statistical Package for Social Science (SPSS) software we could asses on how information in TDM form that should been filled by the medical officers before requesting the TDM test are significant to the outcome result that included some elements as mention above. Indeed, this result will be fully used soon to overcome and improvise this medical utilized factor, which may be frivolous by other physician but extremely important need by the pharmacist. Thus, the government under pharmacy board should come out with some measure to overcome this.
