Abstract
Medication Reconciliation (MR) is defined as the process of scrutinizing patient's prescription that prescribed either in inpatient care settings or outpatient care setting and might be at different time points by reducing the medication discrepancies in order to finalize drug preference for the patient and prevent medication error. Numerous studies show that patient non-adherence, polypharmacy, adverse drug effects and prescriber lacking basic knowledge, caused the medicines given to be left unused. Hence, this is the main factor of medication waste which could lead to a rise of medication discrepancy incident. The aim of this research is to assess the medication reconciliation practice at Outpatient Pharmacy (OPD), Hospital Putrajaya (HPJ). The objectives of this research was to unveil and compare the total medication reconciled according to pharmacological class, correlate the relationship between the age of patients and total medication reconciled per person and to reveal the effectiveness in term of cost-saving from MR practice at OPD of HPJ. The research is a retrospective study, which was done according to the hospital's database by using stratified sampling method. The data collected consist of statistics from 1st January 2016 till 31st December 2016. The data comprised of date, patient's medical record number (MRN), demographic data, quantity drug reconciled per unit, the frequency of MR (referring to how many MR did by the pharmacist), the total cost of drug reconciled, pharmacological class, months and reasons for reconciliation. From this study, about RM62,25 l.49 cost of the drug can be saved from the reconciliation of 127,929 units of the drug from various pharmacological class. This amount could lead to a cost-saving for other hospital's expenses.
