Abstract

The specific medication safety initiatives have been identified and applied according to reduce medication errors. This study focuses on the effectiveness of High Alert Medication Safety Initiatives to reduce the medication error during filling the medicine. Our study was done at Hospital Tengku Ampuan Rahimah, Klang and focussing at Out-Patient Pharmacy. This study seems to be important to assess the statistic of filling errors before and after high alert medication safety initiatives are applied from the survey done at out-patient pharmacy. Our study subject was the filling errors occurred based on the type of error during prescriptions filling at out-patient pharmacy, Hospital Tengku Ampuan Rahimah, Klang on July until December 2015. One or two persons have been in charged on double checked the filled prescription before dispensing to patient and the persons will fill the survey form that we prepared as the filling errors are detected. The focus of the study is to relate the statistic before and after applying the safety initiatives with the effectiveness on reducing filling errors. From the data we managed to collect which were 3 month before and 3 months after safety initiatives were applied, we analyse the data and convert into bar form by using Microsoft. Expectedly, the amount of filling errors occurred after applying the safety initiatives was reduced and the percentage of reduced is 17.1%. Instead, this result will be fully used soon on how to reduce filling errors during prescription filling. As conclusion, High Alert Medication Safety Initiatives are effective on reducing filling errors during prescription filling process. Thus we can strive to reduce and in the future, eliminate any incidence of medical errors towards the patient.
