Factors contributing to medication errors in Hospital Universiti Sains Malaysia (HUSM) Kubang Kerian, Kelantan: pharmacist and pharmacist assistant’s perspective

Medication error is a failure in the treatment process that leads to or causes harm to the patient. This study focuses on the knowledge about medication error and perspective of pharmacists and pharmacist assistants about the contributing factor of medication error. Our study was done in Hospital Universiti Sains Malaysia (HUSM) Kubang Kerian, Kelantan. Our study subjects were pharmacists and pharmacist assistants from the pharmacy department. They were chosen randomly from several units in the pharmacy department and were given a set of questionnaire to be answered. The focus of this study is to know which factor contributes the most in medication error and the knowledge of medication error. 93 filled questionnaires were collected, and the data was analysed in SPSS version 23.0 software to get the result in table and graph form. T test and Levene test were used to find which factors that contributing to medication errors based on pharmacists and pharmacist assistants perspective. From our finding, management related factor and personal related factor have an association in causing medication errors from the perspective of pharmacists and pharmacist assistants while working environment does not have an association with medication error.
