Abstract

‘Kenali risiko lemah jantung’: A Primary Care and Cardiology Public Screening Programme at HASA. Formed through an interdisciplinary collaboration between primary care physicians and cardiology specialists, this community health initiative aims to advance early detection, risk stratify cardiovascular disease, and enhance public awareness of heart failure within the community. Evaluation of the screening program reveals three key contributions: (1) Enhancement of Public Cardiovascular Health Literacy, educating participants on identifying early clinical signs and risk factors associated with heart failure; (2) Implementation of Targeted Multi-Tiered Screening Protocols, utilizing primary care triage alongside specialized cardiological assessments for early diagnosis; and (3) Strengthening of Integrated Referral Pathways, bridging primary care delivery with tertiary cardiology services at Hospital Al-Sultan Abdullah (HASA) to ensure timely intervention and long-term disease management. Overall, this initiative serves as a vital model for preventive cardiology and primary care integration in mitigating the burden of cardiovascular diseases.
