Abstract
This article addresses the critical shift from isolated clinical interventions to a multisectoral, unified approach in combating the HIV epidemic, operating under the paradigm that HIV prevention is a collective societal responsibility. Despite significant advancements in biomedical tools—such as Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis (PEP), and effective Antiretroviral Therapy (ART) that achieves Undetectable = Untransmittable ($U=U$) status—deeply entrenched systemic silos within public health, social services, education, and legal frameworks continue to impede optimal prevention outcomes. This paper analyzes the structural barriers that sustain these silos, notably persistent social stigma, systemic discrimination, and fragmented healthcare delivery models. By advocating for a collaborative framework that integrates community-led organizations, corporate stakeholders, policymakers, and general healthcare practitioners, the study demonstrates that breaking institutional silos fosters a more inclusive, stigma-free environment. Ultimately, the integration of public health strategies with social justice initiatives is shown to maximize the reach of preventative care and accelerate the reduction of new HIV transmissions.
